Police are asking for help in finding a missing toddler who was last seen with his grandmother.

Officers say 1-year-old Isaac Hernandez was left with Leticia Reyes De Lopez on December 30. Since then, Isaac's mother hasn't been able to get a hold of Leticia or seen her son.

Police are concerned about Hernandez and De Lopez's welfare.

Hernandez is described as a Hispanic male with black hair and brown eyes. He was last seen wearing an orange onesie with "Mommy's Little Monkey" on it, along with dark grey pants and a dark blue hoodie with a dinosaur image on it. Police say the child does not walk and he has a scar on the upper/middle section of his back.

Reyes De Lopez is described as a 60-year-old 5' 02" Hispanic female with black hair, brown eyes and weighing 140 pounds.

Police say Reyes De Lopez was last seen leaving with her grandson in the area of 35th Avenue and Northern in Phoenix.
