Newly released video from the National Transportation Safety Board (NTSB) shows the terrifying moments the engine flew off a UPS plane that crashed in Louisville, Kentucky, in 2025.

The surveillance video, which was slowed down to half speed, shows the left engine and left pylon separating from the left wing after the plane was lifting off the runway.

Dig deeper:

Earlier this month, investigators looking into the deadly crash that took place at Louisville’s Muhammad Ali International Airport in November found previous records showing cracks in the engine mount and similar flaws that were discovered during maintenance checks.

Most of those flaws were not reported to the FAA.

The NTSB focused on why no one in government or the industry spotted the concerning trend and took action to prevent it before last November’s crash.

Investigators also released more than 2,000 pages of related documents during a two-day hearing in Washington to examine the disaster’s root causes.

The backstory:

The engine separated from the MD-11’s left wing as it accelerated down the runway.

The crash killed all three pilots on the plane and 12 people on the ground. Twenty-three more were injured. There has only been one other crash involving a similar plane model when the engine fell off.
